Job Summary:
We are seeking a skilled and compassionate Private Duty Nurse LVN to join our team at Quality Talent Group. As a Private Duty Nurse LVN, you will provide high-quality patient care in the comfort of their own homes, working closely with our patients and their families to ensure the best possible outcomes.
Responsibilities:
- Implement and manage patient care plans, working closely with physicians and other healthcare professionals to ensure comprehensive care.
- Conduct thorough patient assessments, identifying and addressing any changes in condition or needs.
- Administer medications, treatments, and therapies as prescribed by physicians, ensuring accurate and timely documentation.
- Provide education and support to patients and their families, promoting health and wellness and empowering them to take an active role in their care.
- Collaborate with our multidisciplinary team to ensure seamless care coordination and communication.
- Document all patient interactions, assessments, and care provided, maintaining accurate and up-to-date records.
Requirements:
- Current, unrestricted license as a Licensed Practical Nurse (LPN/LVN) in the state of practice.
- Current CPR certification from the American Red Cross or American Heart Association.
- One year of experience as a Licensed Nurse in a clinical care setting, with a focus on home health or skilled nursing.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
- Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities.
