Job Description

The Client Advocate Manager is responsible for providing high-quality case management services to our clients. This includes coordinating care, assessing client needs, and developing service plans to ensure the best possible outcomes. The ideal candidate will have excellent communication and interpersonal skills, as well as the ability to work effectively with multiple stakeholders.

**Key Responsibilities:**
  • Collaborate with healthcare providers to coordinate care and develop comprehensive service plans
  • Conduct thorough assessments of client needs and create individualized plans to address those needs
  • Provide education and support to clients and their families to promote self-advocacy and independence
  • Maintain accurate and up-to-date records of client interactions and services provided
  • Participate in interdisciplinary team meetings to discuss client care and develop strategies to improve outcomes

Qualifications:Bachelor's degree in a related field required; Master's degree preferred. Two years of experience in case management or a related field. Excellent communication and interpersonal skills. Ability to work effectively in a fast-paced environment.
