About the Role:
We are seeking a highly skilled and compassionate Healthcare Navigator to join our team at WellSpace Health. In this role, you will play a critical part in supporting patients with obtaining income, insurance, and housing. Your expertise in case management services will make a significant difference in the lives of our patients.
Responsibilities:
- Meet with each patient individually and conduct an intake needs assessment.
- Manage a minimum of 50 patient caseload, providing support and guidance throughout their care journey.
- Write and enter progress notes daily into electronic health records (EHR) systems to maintain accurate patient files.
- Perform 30% of duties in office and 70% of duties traveling to conduct in-person patient visits.
- Manage work schedule when meeting with patients, ensuring timely and efficient communication.
- Represent and/or advocate for patients in obtaining identification, housing, benefits services, and entitlements from governmental and private agencies and organizations.
- Collaborate with patients in developing their individualized treatment plans.
- Rotate after-hours on-call phone duties for a week at a time, including weekends.
- Participate in care coordination meetings with various partners as deemed appropriate.
Requirements:
- A High School Diploma or equivalent is required; an Associate Degree with a focus on social services is preferred.
- Ability to work collaboratively with others in a team environment, valuing diverse perspectives and experiences.
- Maintain appropriate roles and boundaries among patients, peers, and WellSpace Health stakeholders.
- Demonstrate flexibility and adaptability in a fast-paced environment.
- Possess strong communication and listening skills, both verbally and in writing.
- Proficient in Microsoft Office applications, including Word, Excel, and Outlook email.
- Hold a valid driver's license with an insurable driving record that meets our organization's insurance policy requirements.
