Key Responsibilities
  • Provide speech and language therapy services to children in their homes, utilizing a team-based approach to meet the unique needs of each child.
  • Collaborate with other healthcare professionals, including occupational therapists, physical therapists, and service coordinators, to ensure comprehensive care for each child.
  • Develop and implement individualized treatment plans, incorporating evidence-based practices and family-centered care principles.
  • Conduct assessments, evaluations, and progress monitoring to inform treatment decisions and ensure optimal outcomes.
  • Communicate effectively with families, caregivers, and other healthcare professionals to ensure seamless care coordination and family engagement.
Requirements
  • AZ licensed Speech Language Pathologist with experience in pediatric speech therapy and home health settings.
  • Ability to work independently and as part of a team, with excellent communication and interpersonal skills.
  • Strong clinical skills, with a focus on evidence-based practices and family-centered care.
  • Flexibility to work in a variety of settings, including homes, clinics, and community-based locations.
