We are seeking a skilled Nurse Practitioner or Physician Assistant to join our team in post-acute care. As a member of our team, you will have the opportunity to work in a supportive environment with a focus on quality over quantity.
Responsibilities- Conduct daily morning rounds and medical management in top-ranked facilities
- Support a team of medical professionals in a collaborative and dynamic environment
- Manage complex patients with chronic medical conditions such as hypertension, diabetes, CVA, CHF, and dementia
- Order labs and diagnostics as needed, document progress notes, and attend interdisciplinary meetings
- Communicate effectively with patients, staff, and family members
- Master's Degree with a clinical focus on primary care and geriatric medicine
- Physician Assistant or Nurse Practitioner medical license and experience managing complex patients in a hospital or office setting
- National certification
- California state medical license and DEA
- Strong EMR experience and ability to document timely
- Base salary range of $150,000-$170,000
- Monthly bonus structure with increasing percentages over time
- Health insurance, 401K with match, dental and vision insurance, and CME allowance
- PTO and national holidays off, phone reimbursement, and malpractice with tail coverage
