About the Role

Provider Solutions & Development is seeking a highly skilled Advanced Practice Nurse (ARNP) to join our team in a full-time Hospice Care position. As a key member of our interdisciplinary team, you will provide compassionate care to patients and their loved ones in the comfort of their own homes or long-term care facilities.

Responsibilities
  • Perform Face-to-Face Recertification visits and provide symptom management visits as needed
  • Collaborate with a highly skilled and compassionate interdisciplinary team to deliver comprehensive care
  • Provide education and support to patients and their families in formal and informal settings
  • Work a Monday to Friday schedule with 1 in 6 weeks of call, divided into half weeks with paid compensation for each weekend worked
Requirements
  • At least 1 year of ARNP experience, with 2 years preferred
  • Hospice and Palliative experience highly preferred, but not required
  • Ability to work autonomously and as part of a team
  • Strong communication and interpersonal skills
