Job Summary

We are seeking a highly skilled and compassionate Social Worker to join our team at Carle Health. As a Social Worker, you will play a critical role in providing psychosocial services to patients and families within the Emergency Department, helping to identify and address their needs, and connecting them with community resources.

Key Responsibilities
  • Provide psychosocial assessments, supportive counseling, and trauma-informed care to patients and families
  • Collaborate with the healthcare team to develop and implement care plans that address patients' psychosocial needs
  • Connect patients with community resources, such as housing, meals, and support groups
  • Provide emotional support and listening to patients and families
  • Work with security and spiritual care to confirm and notify next of kin
  • Respond to all leveled traumas, victims of violence, and mental health patients in the ED
Requirements
  • Master's Degree in Social Work or Bachelor's Degree in Social Work
  • Licensed Clinical Social Worker (LCSW) Illinois or Licensed Social Worker (LSW) Illinois
  • Ability to coordinate multiple projects concurrently and establish priorities
  • Strong planning and organizational skills
  • Self-motivated and flexible
