The MOBE Clinical Pharmacist provides telephonic Comprehensive Medication Management services to MOBE Participants, identifying, preventing, and resolving medication therapy-related problems. This involves collaborating with MOBE Guides to provide personalized education and information to Participants and their providers, promoting self-management principles and empowering members to achieve optimal medication-related outcomes.
Key Responsibilities- Provide telephonic Comprehensive Medication Management services to MOBE Participants to optimize their medication experience.
- Collaborate with MOBE Guides to provide the best impact for Participants.
- Provide personalized education and information to MOBE Participants and their providers, using motivational interviewing techniques to promote self-management principles.
- Stay abreast of clinical guidelines and research developments for drug, dietary supplement/vitamin, and herbal use.
- Contribute to projects related to data analysis, program and process development, medical writing, and clinical educational content development.
- Precept pharmacy students and student interns.
- Obtain and maintain state licensures, depending on business need.
- Assume Pharmacist in Charge (PiC) responsibilities, with administrative support, based on specific state needs.
- Pharmacy degree from an ACPE-accredited institution and direct experience providing ambulatory care services.
- PGY1 residency or equivalent (3 years) direct patient care experience, with enhanced MTM or CMM experience preferred.
- Board Certified in Pharmacotherapy (BCPS), Ambulatory Care (BCACP), or Psychiatry (BCPP) preferred, but not required.
- Pharmacy licensure in MN or other state, with successful attainment of MN license within 3-6 months of hire, in good standing.
- Willingness to obtain and maintain multi-state licensure(s), and/or PiC status for various states, as determined by business need.
