Join the Department of State Hospitals as a Clinical Social Worker - Community Integration Specialist and play a vital role in the care and treatment of patients. As a member of our interdisciplinary team, you will work closely with community agencies to advocate for the discharge and ongoing treatment needs of patients.
Key Responsibilities:- Develop and maintain effective relationships with community agencies to ensure seamless transitions for patients.
- Provide consultation and functional assistance for complex discharges, ensuring patients receive the necessary support and resources.
- Serve as a resource to staff and patients regarding community integration options, providing education and guidance on available services.
- Coordinate and facilitate treatment groups, promoting patient engagement and participation in their care.
- Collect and analyze data to inform program evaluation and performance improvement recommendations for the department.
- Master's degree in Social Work from an accredited school of social work.
- Registration with the California Board of Behavioral Science as either ACSW or LCSW.
- Minimum of 1 year of work experience as a Clinical Social Worker in a mental health facility.
As a Clinical Social Worker - Community Integration Specialist, you will work in a fast-paced environment, collaborating with various stakeholders to ensure the best possible outcomes for patients. You will be required to follow current face covering guidance as issued by CDPH and may have opportunities for telework, subject to departmental discretion.
Please note that this position requires a Drug Screening, Background Investigation, Medical Clearance, and a valid ACSW or LCSW License. You will be required to provide proof of licensure prior to being hired.
