About the Role

We are seeking a highly skilled and compassionate Registered Nurse to join our Street Medicine team at LifeLong Medical Care. As a Street Nurse, you will play a critical role in providing primary care services to individuals experiencing homelessness in our community.

Key Responsibilities
  • Provide nursing care outreach in encampments and streets to highly-vulnerable individuals experiencing homelessness.
  • Implement care plans and provide urgent and primary care services within the Registered Nurse scope of practice.
  • Collaborate with medical assistants and other support staff to coordinate care and develop strong relationships with patients.
  • Participate in Street Medicine clinical case conferences and other case conferences and meetings.
Requirements
  • Current and valid California Registered Nurse license and BLS.
  • Proficient in Microsoft office suite and electronic health records, Epic preferred.
  • Minimum of one year experience in community health as an RN.
  • Possession of current driver's license and insurance.
Preferred Qualifications
  • Bachelor's Degree in Nursing.
  • Lived experience relative to working with people experiencing homelessness.
