Job Summary

We are seeking a compassionate and skilled Licensed Practical Nurse to join our team at EverHeart Hospice. As a LPN, you will play a vital role in providing exceptional end-of-life care to patients and their families.

Responsibilities
  • Conduct home visits to provide hospice services and support to patients and their loved ones.
  • Respond to referrals and inquiries related to hospice care, its benefits, and palliative care.
  • Collaborate with medical professionals to obtain necessary orders and ensure seamless care coordination.
  • Manage patient data, including medication orders, DME, and transportation arrangements.
  • Support with paperwork, documentation, and data entry to ensure accurate and timely information.
  • Review patient admissions for accuracy and completeness.
  • Follow up with patients enrolled in our In Touch Program.
  • Assist with pre-admission tasks, including referrals, medication orders, and transport arrangements.
  • Handle referrals and pre-admission communications.
  • Gather patient health information and input necessary data into our EMR system.
  • Support with data entry, including physician orders, frequency orders, medication, and DME.
  • Assist with QAPI audits and ensure compliance with Agency policies and regulations.
Qualifications
  • Graduation from an approved School of Practical Nursing.
  • Current LPN licensure in the State of Ohio.
  • Minimum of one year of experience in Medical/Surgical, Oncology, or community health nursing.
  • Hospice experience preferred.
  • Shall have a valid driver's license.
  • Excellent customer service skills.
  • Detail-orientated with excellent communication, documentation, and organizational skills.
  • Ability to work independently without supervision.
  • Travel to other sites as needed. Travel may be in inclement weather.
  • Ability to lift 25 lbs. of supplies or equipment.
  • Requires various amounts of sitting, standing, twisting, turning, bending, lifting, and walking.
  • Weekend and Holiday rotation.
