Responsibilities
  • Provide skilled nursing services, including Infusion, Wounds, and other patient care needs
  • Conduct home evaluations and develop individualized care plans
  • Collaborate with healthcare providers and caregivers to ensure seamless care coordination
  • Provide education and resources to patients and caregivers to manage conditions and remain in their own homes
Requirements
  • Graduate of an accredited nursing program
  • Minimum one year experience as a professional nurse within the last three years
  • Ability to communicate effectively and work independently
  • High level of organization, responsibility, and self-management
  • Active RN license in the state of Missouri and BLS certification
