About the Position

As a Nurse Practitioner - PRN/Part Time, you will conduct home-based assessments, medical history, diagnosis, and treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols.

You will work collaboratively with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate. You will also collaborate with Primary Care Physicians (PCPs) on patient education and provide follow-up.

You will provide services in a variety of venues, including home visits, skilled facility visits, and telehealth visits.

Responsibilities

Responsibilities include:

  • Conducting adult/geriatric assessments, medical history, diagnosis, and treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols.
  • Working collaboratively with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate.
  • Collaborating with Primary Care Physicians (PCPs) on patient education and providing follow-up.
  • Providing services in a variety of venues, including home visits, skilled facility visits, and telehealth visits.

Nurse Practitioner - PRN/Part Time - Home Risk Assessment

Type: PRN/Part Time - Flexible-Nurse Practitioner

PAY: $90.00 per completed Assessment

Location: Home, SNF, Telehealth, and other community environments in Los Lunas, New Mexico

Hours: Days, Evening, Nights, weekend, Flexible Scheduling

Benefits: PRN/Part Time providers are non-benefited except for 401(k) and limited Voluntary Benefits

Mileage Reimbursement plan

Qualifications

Must Haves:

  • Master's Degree required OR commensurate experience and satisfactory completion of NP licensure.
  • Current RN and NP licensure in the state of practice to include prescription authority or the ability to obtain prescriptive authority.
  • Board certified by the AANP, ANCC, or the AACCN in a Matrix-approved specialty.
  • Have a current BLS, ACLS, or CPR certification.
  • Have a Minimum of 1 year experience as a Nurse Practitioner.
  • Have strong computer skills and familiarity with Employee health/medical record software.
  • Possess excellent verbal and written communication skills with patients, clients, and colleagues.
  • Be comfortable and flexible with frequent change.

Travel Requirements:

  • Have the ability to travel, a valid state driver license, and able to drive a car, proof of adequate automobile insurance coverage for the state of residence.
  • Travel required to meet patients where they live.
