Job Title: Home Health Physical Therapist

BAYADA Home Health Care is seeking a highly skilled and compassionate Home Health Physical Therapist to join our team in Thornton, Pennsylvania.

Job Summary:

We are looking for a dedicated and experienced Physical Therapist to provide high-quality care to our clients in the comfort of their own homes. As a Home Health Physical Therapist, you will be responsible for assessing and treating clients with various physical and mobility limitations, developing and implementing individualized care plans, and educating clients and their families on rehabilitative care and activities.

Responsibilities:
  • Conduct thorough assessments of clients' physical and mobility limitations
  • Develop and implement individualized care plans to address clients' needs and goals
  • Provide manual therapeutic exercises, gait training, balance, and other interventions as needed
  • Educate clients and their families on rehabilitative care and activities
  • Accurately document observations, interventions, and evaluations in a state-of-the-art touch pad tablet
Qualifications:
  • Current Pennsylvania Physical Therapist license
  • Minimum of one year of recent work experience as a Physical Therapist
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association
  • Ability to work independently and manage time effectively
  • Strong interpersonal, organizational, and problem-solving skills
  • Solid computer skills; prior experience with electronic medical records (EMR) preferred
