This role involves providing compassionate nursing care in a hospice setting, primarily focusing on home health services. This position operates on a pay-per-visit basis, requiring travel to various patient locations.
Qualifications:
- Associate degree in nursing or higher
- Minimum of 1 year of experience in any of the following areas:
- Clinical Documentation
- Clinical Informatics
- Clinical Quality
- At least 1 year of experience with Electronic Medical Records (EMR)
- Current Registered Nurse (RN) license in Pennsylvania (PA)
- Certification in ACLS or BLS from the American Heart Association
- Willingness to travel as required for the role
Key Responsibilities:
- Lead and collaborate in the management and delivery of patient care plans
- Provide support to patients in various locations
- Allocate approximately 20% of the workday for travel to and from patient sites
- Ensure timely completion of charting and documentation
- Effectively manage personal time and scheduling
Work Schedule:
This position requires on-call availability during weekends and holidays, with specific hours designated for patient care.
Travel Requirements:
Local travel is a necessary component of this role.
