Nurse Practitioner

As a Nurse Practitioner in our Home Health Care team, you'll conduct in-home assessments, including evaluations of past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical exams. You'll also perform evidence-based practice screenings, including point-of-care testing (as appropriate), and identify diagnoses to be used in care management and active medical management in the furtherance of treatment.

Responsibilities:- Conduct in-home assessments and evaluations
- Perform evidence-based practice screenings and identify diagnoses
- Develop and implement care plans to improve patient outcomes
- Communicate with patients, families, and healthcare teams to ensure seamless care coordination
- Travel up to 85% to visit patients in their homes
- Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care certification
- Licensure must be unencumbered, free of any open/unresolved disciplinary actions
- Ability to complete physical requirements of the job, including lifting a 30-pound bag in and out of the car
- Experience in gerontology, cardiology, internal medicine, or endocrinology
