About the Role

ChenMed is seeking a highly skilled and experienced Medical Director to lead our Transitional Care team. As a key member of our leadership team, you will be responsible for developing and implementing strategies to improve patient outcomes, reduce hospital readmissions, and enhance the overall quality of care.

Key Responsibilities
  • Lead the Transitional Care team in developing and implementing care plans for patients with complex medical needs.
  • Collaborate with primary care physicians, hospitalists, and other healthcare professionals to ensure seamless transitions of care.
  • Develop and maintain relationships with external organizations, such as hospitals and skilled nursing facilities, to ensure effective communication and coordination of care.
  • Monitor and analyze patient outcomes, identifying areas for improvement and implementing strategies to address them.
  • Develop and implement quality improvement initiatives to reduce hospital readmissions and improve patient satisfaction.
  • Provide leadership and guidance to the Transitional Care team, ensuring that they have the necessary resources and support to deliver high-quality care.
Requirements
  • Medical degree (MD or DO) and current, active licensure in the state of employment.
  • Board certification in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty.
  • At least three years of post-residency experience, with a minimum of two years in a leadership role.
  • Strong clinical skills in the management of seniors with complex medical needs.
  • Excellent leadership and communication skills, with the ability to build relationships with external organizations and healthcare professionals.
  • Ability to analyze data and implement quality improvement initiatives.
  • Fluency in English, both verbally and in writing.
Preferred Qualifications
  • Experience in quality improvement and medical leadership activities.
  • Previous experience working with interdisciplinary teams.
  • Knowledge of information technology applications in medical settings.
