About the Role:

The successful candidate will provide primary and urgent care services to nursing center residents in collaboration with other medical providers. This is a full-time position (40 hours per week) with a competitive hourly rate of $54-$60.

Responsibilities:
  • Provide high-quality patient care and education to residents and their families
  • Collaborate with the Center Leadership Team to achieve AlignMed Partners' mission of delivering exceptional healthcare and compassion
  • Work closely with the Medical Director and/or Attending Physician to ensure seamless patient care
  • Participate in ongoing training and professional development to stay up-to-date with best practices
Requirements:
  • Master's Degree in Nursing with a concentration in Older Adult and/or Family Nurse Practitioner
  • Minimum of two years' experience working with adult or geriatric populations
  • Unrestricted professional license and DEA in states where practice allows
