Advanced Primary Care Nurse Practitioner- West Chester, PA Position

We are seeking a dedicated and experienced Registered Nurse Practitioner to join our team of medical professionals. This Full-Time Permanent position is available in West Chester, PA.
The successful candidate will provide direct patient care, discussing symptoms and concerns, and perform thorough examinations to diagnose medical problems and prescribe medication or create treatment plans as necessary. Additional responsibilities include recording medical histories, administering diagnostic tests, and collaborating with physicians when required.
To excel in this role, the ideal candidate should possess excellent communication skills, be empathetic but resolute in decision-making, and exhibit cultural competence and teamwork skills.
Key Responsibilities:
- Perform physical examinations and screenings of patients.
- Record patient medical histories and symptoms.
- Create patient care plans and contribute to existing ones.
- Order, administer, and analyze diagnostic tests, including bloodwork.
- Review and interpret test results to diagnose health problems.
- Recommend further treatment options or future appointments if needed.
- Monitor and operate medical equipment.
- Administer medicine and other treatments.
- Provide referrals to other practitioners when necessary.
- Detect changes in patient health and modify treatment plans accordingly.
- Offer after-care/ongoing care information and resources to patients and their families.
- Consult with healthcare professionals and families.
- Train patients and their families to manage and prevent illness and injury.
- Maintain accurate and up-to-date patient records.

This position requires a minimum of 1 year of recent experience as an RN in a clinical setting. Active RN licensure within the state of practice and current certifications are also necessary. Additionally, prescriptive authorization and knowledge of computer-based data management programs are essential requirements.
