Position Overview:
The focus of this role is to manage daily patient care within a primary care health facility under the guidance of a Registered Nurse/Clinical Manager and healthcare provider.

Key Responsibilities:
1. Conduct initial assessments of patients to determine the urgency of their medical issues.

2. Escort patients to examination rooms and ensure that all relevant medical records and necessary supplies are ready for the provider.

3. Gather and document comprehensive initial data, including medical history and vital signs, prior to the patient's consultation with the provider.

4. Assist the provider during patient examinations and treatments as needed.

5. Perform authorized laboratory tests and accurately record results in designated files.

6. Conduct EKG tests as required.

7. Ensure examination rooms are adequately stocked and maintain cleanliness after each patient visit.

8. Monitor health center supplies and compile lists of items that need to be ordered.

9. Schedule diagnostic tests for patients as directed by the provider.

10. Educate patients on medication, dietary needs, health maintenance, and diagnostic procedures.

11. Record and follow provider orders diligently.

12. Prepare specimens for external laboratory testing.

13. Document interactions in the Electronic Health Record (EHR).

14. Ensure patient comfort and well-being during their visit.

15. Assist the provider with various procedures as necessary.

16. Administer medications to patients in accordance with provider instructions.

17. Prepare sterile materials for medical use as required.

18. Dispose of disposable materials following sanitation protocols.

19. Perform nursing tasks such as dressing changes and ear irrigation as prescribed by the provider.

20. Support care coordination, which may involve obtaining test results and communicating with various entities, including community organizations and specialists.

21. Proactively address the needs of patients and their families through effective population management strategies.

22. Communicate effectively with all patients, particularly those from vulnerable populations.

23. Provide self-management support through patient coaching and motivational interviewing when appropriate.

24. Recognize the importance of teamwork in delivering patient care.

25. Collaborate and communicate effectively while providing patient care.

26. Actively engage in the medical home care team, which includes both clinical and non-clinical staff working together to achieve a common goal: delivering high-quality care to patients.

27. Adhere to the organization's confidentiality policies in compliance with the Health Insurance Portability and Accountability Act (HIPAA).

28. Perform additional duties as assigned.

Work Environment:
This position requires manual dexterity for operating standard office equipment. Hearing must be within the normal range for human conversation.

This role demands the ability to maintain focus and attention to detail for extended periods to ensure accurate record-keeping related to patient and account information.

The position operates within a standard office environment.

Qualifications:
Graduate of an accredited LPN program. Must hold and maintain appropriate state licensure. Strong organizational skills and the ability to multitask are essential. A friendly demeanor with a genuine desire to work with the public is required.
