Job Description:

We are seeking a skilled and compassionate Registered Nurse (RN) to join our team as a Case Management specialist. In this role, you will work collaboratively with healthcare professionals to assess, plan, facilitate, and coordinate care for individual patients. Your expertise will be invaluable in promoting patient safety, quality of care, and cost-effective outcomes.

Required Skills and Qualifications:

  • At least 1 year of recent acute care experience in specialty
  • Active RN license
