Home Care Licensed Practical Nurse (LPN) Job Opportunity

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team in Marinette, WI. As a Home Care LPN, you will provide high-quality nursing care to patients in their homes, working collaboratively with our healthcare team to deliver exceptional patient outcomes.

Key Responsibilities:
  • Provide direct nursing care to patients in their homes, following established protocols and procedures.
  • Conduct thorough assessments of patients' physical and emotional needs, developing and implementing individualized care plans as needed.
  • Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
  • Document patient visits and care activities in a timely and accurate manner, adhering to regulatory and payer requirements.
  • Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements:
  • Current LPN licensure in the state of practice.
  • Wisconsin LPN license or a Multistate license.
  • Current CPR certification.
  • Previous home care experience preferred.
