Job Description:

The successful candidate will perform nutritional assessments and ongoing evaluations of the plan of care for each resident, providing personalized guidance and support to residents and their families. They will also participate in IDCP care conferences as needed, ensuring seamless communication among healthcare teams.

Key Responsibilities:

  • Conduct thorough nutritional assessments to identify individual needs
  • Develop and implement personalized nutrition plans tailored to residents' requirements
  • Collaborate with interdisciplinary teams to ensure comprehensive care

Required Skills and Qualifications:
