Key Responsibilities:
- Assist with the pre-admission and admission processes, helping clients transition smoothly into our facility.
- Conduct interviews with residents and their families to gather relevant information regarding their backgrounds and needs.
- Document progress and update care plans during interdisciplinary care conferences.
- Coordinate transportation and appointments, serving as a liaison among residents, families, and external agencies.
- Provide support through individual counseling and group sessions during the placement process.
- Evaluate potential discharge plans upon admission and arrange for necessary follow-up services.
- Assess the emotional, financial, spiritual, educational, and social needs of residents, ensuring these are addressed through our facility or external resources.
- Act as an advocate for residents and their families, ensuring they are informed of their rights.
- Organize and facilitate Family Council Meetings, maintaining accurate records of discussions.
- Maintain effective communication with residents, families, healthcare professionals, and facility staff.
Qualifications:
- Bachelor's or Master's Degree in Social Work.
- Valid State License.
- Minimum of one year of supervised social work experience in a healthcare environment, working directly with patients and families.
- Experience in healthcare settings, preferably in nursing homes, hospice, home health, or hospitals.
- Ability to collaborate with diverse individuals in various community and healthcare planning scenarios.
- Proficient in making informed decisions based on evaluations and interpretations of case histories.
