Job Summary

As a skilled Home Health RN, you will provide coordinated care to patients in the comfort of their own homes. This role requires a strong understanding of patient needs and the ability to work collaboratively with the interdisciplinary team to establish and achieve patient goals.

Responsibilities
  • Assess patients' physical, psychosocial, and environmental needs, identifying areas for improvement and developing personalized care plans.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Supervise and provide clinical direction to home health aides and LPNs/LVNs to ensure quality and continuity of services.
  • Participate in on-call rotation and emergency call according to agency policy.
Requirements
  • Graduate of an accredited school of professional nursing.
  • Minimum of one or more years of home health, public nursing, or acute hospital nursing experience.
  • Familiarity with Medicare home health regulations, documentation requirements, ICD-10 coding, and PPS.
  • Reliable transportation and proof of valid automobile liability insurance.
  • Current BCLS Certification prior to providing patient care.
