- Document patient information, including vital signs and medication therapies, in accordance with agency and regulatory requirements.
- Facilitate individual and group training sessions to promote basic living skills and independence.
- Model appropriate behavior and interventions, promoting a positive and respectful environment.
- Monitor clients for unusual or harmful behaviors, reporting observations to the House Supervisor.
- Assist with coordinating client activities, both in-house and in the community.
- Communicate effectively with clients, families, and peers, both verbally and in writing.
- Transport clients to appointments and activities, ensuring safe and timely arrival.
- Regularly review house rules with clients, promoting a sense of responsibility and accountability.
- Maintain a clean and safe environment, adhering to agency and regulatory standards.
- Participate in fire drills, emergency procedures, and safety strategies, ensuring a prepared and responsive team.
- Report concerns and issues to the House Supervisor and Facility Manager, promoting a culture of open communication.
- Attend meetings and training sessions as required, staying up-to-date on agency policies and procedures.
- Understand and carry out verbal and written instructions, demonstrating a commitment to teamwork and collaboration.
- Maintain confidentiality, respecting the privacy and dignity of clients and colleagues.
- Report to shift as scheduled, following call-out procedures when necessary.
- Report any facility or van issues to the House Supervisor and Facility Manager, ensuring prompt attention and resolution.
- Complete all other duties and tasks assigned by Leadership, demonstrating a flexible and adaptable approach.
- High School Diploma or equivalent.
- Basic computer skills and proficiency in electronic health records (EHRs).
- Excellent oral and written communication skills, with the ability to communicate effectively with diverse populations.
- Excellent 5-year driving record, with a valid driver's license.
- Certification in CPR/First Aid, MAC I and II, and CPI, or ability to obtain within a specified timeframe.
- Personal automobile liability insurance, with minimum coverage of $100,000, $300,000, and $50,000.
