Job Summary

We are seeking a skilled and compassionate Hospice Nurse to join our team. As a Hospice Nurse, you will play a vital role in providing palliative care to terminally ill patients and their families, ensuring their physical, emotional, and spiritual needs are met.

Responsibilities

  • Assess and admit patients to our home hospice programs, ensuring they receive the highest level of care.
  • Develop and implement individualized plans of care in collaboration with patients, families, physicians, and other members of the care team.
  • Provide education and support to patients and their families, empowering them to make informed decisions about their care.
  • Maintain accurate and comprehensive documentation in patient health records, adhering to regulatory requirements.
  • Collaborate with the care team to ensure seamless transitions and continuity of care.

Requirements

  • RN licensure in the State of Maryland.
  • Current Cardio Pulmonary Resuscitation (CPR) certification.
  • Prior experience in hospice or home health care is strongly preferred.
  • Valid driver's license and reliable transportation.
  • Certified Hospice and Palliative Nurse (CHPN) certification is a plus.
