LPN - St. Lawrence County Healthcare Professional

Job Summary:
We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Arc Jefferson - St. Lawrence. As an LPN, you will play a vital role in providing high-quality healthcare services to individuals with developmental disabilities.
Responsibilities:
- Assist the Registered Nurse (RN) in maintaining current health records, medication, and treatment sheets for assigned individuals.
- Maintain current listings of allergies and special needs, such as special diets, food sensitivities, photo-sensitivity, and tetanus updates.
- Participate in the development of medical services and schedule medical appointments as required by regulation and directed by the RN.
- Attend appointments where significant medical issues will be addressed and ensure that prescriptions, medications, and supplies are ordered, maintained, and documented per Agency Policies.
- Monitor individuals for signs and symptoms of acute illness, communicable diseases, and disease complications/exacerbation of chronic illness, reporting concerns to the site RN and completing documentation per Agency expectations.
- Review T-logs for noted concerns at the beginning of each scheduled shift and report any concerns noted to an RN, documenting status of minor injuries, doctor appointments, contact with the physician, pharmacy, family, nursing, etc.
- Ensure physical plant and staff compliance with sanitation and infection control and assist with the preparation of reports and attend team meetings as requested by the RN.
- Administer first aid and nursing instruction to individuals and staff as needed and provide direct support services to individuals.
- Provide medical input and documentation as indicated on all reportable and serious notable occurrences/situations related to allegations of abuse, injuries, or mistreatment.
- Report any unusual findings, such as bruises, marks, behaviors, to appropriate persons and administer medications per staffing needs, at least once weekly.
- Complete weekly Medication Administration Record (MAR) Review, report medication errors, and document accordingly.
Requirements:
- Licensed and currently registered with the New York State Department of Education as a Licensed Practical Nurse.
- Will accept GPN (Graduated Practical Nurse) with the expectation that the NYS Licensed Practical Nurse License is obtained in 90 days.
- Experience working with developmentally disabled preferred.
- Valid New York State Driver's License.
- Authorized to work in the United States.
Work Environment:
This job operates in the residential environment of the people we support. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets, and fax machines.

While performing the duties of this job, the employee is regularly required to talk or hear. The employee frequently is required to stand; walk; lift; use hands to finger, handle, or feel; and reach with hands and arms. Work may require lifting, pushing, or pulling in excess of 30 pounds.
