We are seeking a highly skilled Wound Care Assistant Team Lead to join our team at Canyon Transitional Rehab Center in Albuquerque, NM. As a key member of our interdisciplinary team, you will play a vital role in developing and implementing a person-centered approach to patient care related to wound prevention and management.
Key Responsibilities- Collaborate with the interdisciplinary team to provide comprehensive wound care services to patients.
- Develop and implement individualized wound care plans to promote optimal patient outcomes.
- Train and educate staff on wound care best practices and protocols.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
- Participate in quality improvement initiatives to enhance wound care services and patient satisfaction.
- LPN licensure
- Completion of Skin Health Team Lead orientation training within 90 days of hire
- Commitment to complete Advanced Wound Training and national accredited Wound Certification within 1 year of hire
- Competitive hourly rate: $34 - $37
- Full-time position
