We are seeking a skilled Case Manager to join our team. In this role, you will provide exceptional patient care by collaborating with multidisciplinary teams to develop and implement effective case management programs.

Responsibilities:
- Lead the coordination of patient care with other disciplines within the care team, ensuring timely and appropriate care.
- Develop and implement case management programs that optimize patient outcomes.
- Serve as a liaison between patients, families, nurses, physicians, and healthcare benefactors.
- Collaborate with acute care case managers to ensure ongoing care appropriateness.
- Coordinate transfers to tertiary centers and collect data for regulatory compliance.
Requirements:
- Bachelor's Degree in Nursing (BSN) preferred.
- Experience in a care management role is desired.
- Registered Nurse (RN) licensure in the state of practice is required.
- Case management certification is preferred.
- Basic Life Support (BLS) or Healthstream Basic Life Support (HS-BLS) is required.
