Job Title: Travel Nurse RN Case Manager

Tact Staff is seeking a skilled travel nurse RN Case Manager to join our team in a dynamic and challenging role.

Job Summary:

We are looking for a highly motivated and experienced travel nurse RN Case Manager to work in our Acute Care Case Management department. As a travel nurse RN Case Manager, you will be responsible for providing high-quality patient care and managing complex case management needs.

Responsibilities:
  • Provide exceptional patient care and manage case management needs in a fast-paced Acute Care environment.
  • Collaborate with healthcare teams to develop and implement effective care plans.
  • Conduct thorough assessments and evaluations to identify patient needs and develop individualized care plans.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Manage and coordinate patient care services, including referrals, authorizations, and follow-up care.
  • Participate in quality improvement initiatives to enhance patient outcomes and care delivery.
Requirements:
  • Current RN licensure and active registration.
  • Minimum 2 years of experience in Acute Care Case Management or a related field.
  • Excellent communication and interpersonal skills.
  • Ability to work in a fast-paced environment and prioritize multiple tasks.
  • Strong analytical and problem-solving skills.
