Job Summary

We are seeking a skilled Registered Nurse (RN) to join our team as a Nurse Care Specialist. This is a rewarding opportunity to work with patients and their families in a compassionate and supportive environment.

Responsibilities:
  • Assess patient needs and develop individualized care plans to ensure optimal outcomes.
  • Collaborate with healthcare professionals to provide high-quality, patient-centered care.
  • Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and implementing targeted interventions.
  • Develop and maintain accurate and detailed patient records, ensuring timely updates and effective communication with healthcare teams.
  • Implement evidence-based practices to promote positive patient experiences and improve health outcomes.
  • Educate patients and their families on disease management, treatment options, and self-care strategies.
Requirements:
  • A current RN license in the state of practice.
  • One year of prior experience in nursing, preferably in home healthcare or gerontology.
  • Excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
  • Ability to lift up to 50 pounds and move freely within patient homes.
Compensation:

The salary range for this position is $28-32 per hour, based on experience and qualifications.
