We are seeking an experienced Care Coordinator Behavioral Health RN to join our team in Swampscott, Massachusetts. As a key member of our interdisciplinary team, you will provide comprehensive care coordination services to patients with complex behavioral health needs.

Key Responsibilities:

  • Provide direct care coordination services to patients with behavioral health needs, including assessing their medical, social, and emotional needs.
  • Collaborate with healthcare providers, families, and community resources to develop and implement individualized care plans.
  • Ensure seamless transitions between care settings, including hospital-to-home and home-to-hospital transfers.
  • Maintain accurate and up-to-date records of patient information, including medical histories, treatment plans, and progress notes.

Requirements:

  • Graduate of an accredited school of Nursing with current licensure as a Registered Nurse (RN) in the Commonwealth of Massachusetts.
  • 2+ years of experience in behavioral health or a related field.
  • Ability to work effectively in a fast-paced environment with multiple priorities and deadlines.
  • Excellent communication and interpersonal skills, with ability to collaborate with multidisciplinary teams.
