Job Summary:
We are looking for a highly motivated and experienced Physical Therapist to provide high-quality patient care in our outpatient rehabilitation services. As a Physical Therapist, you will work closely with our interdisciplinary team to develop and implement individualized treatment plans that promote optimal patient outcomes.
Key Responsibilities:
- Evaluate and re-evaluate patients in a timely manner to assess their physical function and develop effective treatment plans.
- Establish and maintain a safe and supportive environment for patients to receive physical therapy services.
- Collaborate with other healthcare professionals to ensure comprehensive patient care and achieve optimal patient outcomes.
- Document patient progress and maintain accurate records to ensure continuity of care.
- Provide education and training to patients and their families to promote self-management and independence.
Requirements:
- BS, Masters, or Doctorate degree from an A.P.T.A. approved school of Physical Therapy.
- Licensure or eligibility for licensure in Delaware.
