Position Overview:

Key Responsibilities:
1. Evaluate the effectiveness of therapeutic interventions and manage patient follow-up care.
2. Collaborate with physicians to address any questions or concerns regarding patient treatment and care.
3. Maintain comprehensive documentation of all patient interactions in the medical record, whether in-person or via telecommunication.
4. Partner with physicians and colleagues to develop clinical standards, objectives, and safety protocols, while ensuring quality assurance and effective use of clinical resources.
5. Engage with patients through various communication channels, addressing inquiries, discussing medication refills, scheduling appointments, and reviewing lab results.
6. Educate and promote health maintenance practices to patients and their families.
7. Serve as a role model and mentor for clinical staff and other advanced practice healthcare professionals.
8. Participate in educational initiatives for patients, families, and healthcare professionals, including presentations and workshops.
9. Adhere strictly to the clinic's Compliance Plan and all relevant regulatory standards.

Performance Expectations:
- Strong knowledge of cardiac health and related diagnostic and treatment options is preferred.
- Excellent communication skills with patients and families regarding health matters.
