Job Summary:
Oak Street Health is seeking a highly skilled Nurse Practitioner to join our team as a Home Visits Specialist. As a key member of our healthcare team, you will deliver high-quality primary care to our homebound patients, working collaboratively with other healthcare professionals to ensure comprehensive and coordinated care.
Key Responsibilities:
- Conduct medical house calls for homebound patients, following established clinical pathways and guidelines.
- Optimize and monitor patient clinical status, identifying and addressing gaps in care, and reconciling medications and addressing adherence challenges.
- Collaborate with home health, specialists, social workers, hospitals, SNFs, and other healthcare providers to ensure seamless care coordination.
- Ensure clinical excellence, compliance with regulatory guidelines, and promotion of the Oak Street Health care model.
- Perform other duties as assigned.
Requirements:
- Genuine passion for primary care and serving under-resourced communities.
- Strong clinical skills focused on older adults with complex disease states.
- Experience in home-based care (as RN or APN).
- Excellent communication, collaboration, and problem-solving skills.
- Desire to be part of a fast-paced, innovative team.
- Active, non-probationary state medical license, including US work authorization, if applicable.
- 1+ years APN experience, preferably with a Medicare population.
- Access to reliable transportation.
- Flexibility to travel around the local market.
- Bilingual (English and Spanish) preferred, but not required.
- Working knowledge of Word, Excel, and EMRs.
- US work authorization.
