We are seeking a skilled Nurse Practitioner to join our team of healthcare professionals, providing primary health care and patient care management in residential and assisted living facilities.
**Key Responsibilities**
- Deliver high-quality, patient-centered care to vulnerable populations in the comfort of their homes.
- Manage a reasonable daily case load of 6-8 patients via face-to-face visits.
- Collaborate with our interdisciplinary team to ensure seamless care coordination.
- Participate in ongoing education and training to stay up-to-date on best practices in geriatric care.
**Requirements**
- Experience in geriatrics or complex patient care preferred.
- Active and unrestricted license and DEA certification.
- Board certification as a Nurse Practitioner.
- Experience with electronic medical records (EMRs).
