About the Role:- Provide social work services to patients and their families in acute care settings
- Conduct psychosocial assessments and develop individualized plans to address patient needs
- Collaborate with healthcare teams to ensure comprehensive care and effective discharge planning
- Provide crisis intervention and support to patients and families in crisis
- Develop and implement programs to promote patient education and empowerment
- Master's degree in social work from a CSWE-accredited program
- MO LMSW or eligible (must obtain licensure within 90 days)
- Knowledge of social work theory and practice at the MSW level
- Excellent written and verbal communication skills
- Ability to work effectively in a fast-paced, dynamic environment
