Job Summary

We are seeking a compassionate and skilled Certified Nursing Assistant to join our team at Four Seasons Nursing and Rehab. As a key member of our healthcare team, you will provide high-quality patient care and support to our residents, ensuring their comfort, dignity, and well-being.

Responsibilities
  • Provide routine daily nursing care to residents, including assistance with personal hygiene, activities of daily living, and mobility.
  • Monitor and report changes in resident condition to the Charge Nurse in a timely manner.
  • Observe and report presence of skin breakdown to prevent pressure ulcers.
  • Record all care provided in our electronic medical record system.
  • Use wristbands and EMR to identify residents before administering treatments, serving meals, etc.
  • Report all accidents/incidents observed on the shift that they occur.
  • Ensure resident rooms are ready for receiving residents, including making beds.
  • Assist with lifting, turning, moving, positioning, and transporting residents.
  • Answer resident calls promptly.
  • Assist residents with walking, using devices as instructed.
  • Perform nursing rehab procedures, including daily range of motion exercises.
  • Record as indicated.
  • Provide care as instructed.
  • Assist in preparing residents for physical examinations.
  • Check residents routinely to ensure personal care needs are met.
  • Perform foot care and report any findings to the nurse.
  • Prepare residents for meals, including taking to the bathroom, washing hands, combing hair, raising beds, positioning tables, placing bibs, and taking to/from the dining room.
  • Serve food trays and assist with feeding as indicated.
  • Record resident food/fluid intake as instructed.
  • Serve between meal and bedtime snacks.
  • Perform after meal care, including removing trays, cleaning resident's hands, face, clothing, etc.
  • Maintain confidentiality of all resident care information.
  • Ensure all nursing care is provided in privacy.
  • Knock before entering resident rooms and await response.
  • Pack and log all discharge resident belongings.
  • Responsible for cleanliness of resident environments.
  • Make occupied and unoccupied beds.
  • Make bed check rounds on all assigned residents every two hours at night and report status to the Charge Nurse.
  • Prepare laundry as per policy on assigned days.
  • Measure and record temperatures, pulse, and respirations (TPRs) as instructed.
  • Assist residents and family during admission/transfer/discharge.
  • Appropriately communicate with residents, including those with sensory loss, memory, cognitive, or perceptual impairment.
  • Maintain personal belongings, including clothing, dentures, eyeglasses, hearing aids, and prostheses.
  • Assist with maintaining bowel regularity and report absence of bowel movements as per policy.
  • Assist with the care of ostomies, including colostomy and ileostomy, as per nurse's instructions.
  • Recognize and report abnormal signs and symptoms of common diseases and conditions.
  • Care for dying residents, including care of the body and personal effects after death.
  • May perform other duties and assist in areas of the department as directed by the Director of Nursing, Assistant Director of Nursing, Resident Care Coordinator, Administrator, and/or designee.
Requirements
  • Must be in good mental and physical condition.
  • Must be able to move intermittently throughout the workday.
  • Must be able to cope with the mental and emotional stress of the position.
  • Must function independently, have flexibility, personal integrity, and the ability to work with residents, staff, family members, visitors, and support and regulatory agencies.
  • Must be in good general health and demonstrate emotional stability.
  • May be required to lift, push, and move at least 50 pounds.
  • Must be able to assist in the evacuation of residents.
  • Must be able to climb several flights of stairs in an emergency.
  • Must maintain a high level of personal hygiene and cleanliness.
  • Must be able to physically operate nursing home-specific equipment, including hospital beds, Hoyer lifts, century tubs, etc.
Qualifications
  • Must be good moral character.
  • Some geriatric experience is preferable but not essential.
  • Must possess a high school diploma or GED.
  • Must possess a current NYS nurse aide certification and be listed in the New York State RHCF Nurse Aide Registry.
  • Must be able to read, write, and speak the English language in an understandable manner to facilitate effective communication and follow oral and written instructions.
  • Must be willing to cooperate with Resident Care Coordinators, fellow employees, other department heads, and family members.
Working Hours

Works 37 1/2 hours per week or as assigned. May work different shifts, including weekends, as per facility policy.

Working Conditions
  • Works throughout the facility, including nursing units, resident rooms, dining areas, outdoor patio areas, etc.
  • Sits, stands, bends, lifts, and moves intermittently during working hours.
  • Is subject to frequent interruptions.
  • Is involved with residents, personnel, visitors, government agencies/personnel, etc., under all conditions and circumstances.
  • Is subject to hostile and emotionally upset residents, family members, visitors, etc.
  • Works beyond normal duty hours and on weekends in an emergency situation.
  • Attends and participates in continuing educational programs.
  • Is subject to falls, odors, etc., throughout the workday.
  • May be exposed to residents with chronic and acute conditions and/or illnesses.
