Job Summary

The Hospice Registered Nurse will bring the essential elements of the hospice delivery model into the patient's defined living space through assessing, planning and implementing a treatment plan to keep terminally ill patients comfortable.

Key Responsibilities:

  1. Monitor vital signs, administer medications and support patients and their families through this transition.
  2. Coordinate patient care and communicate and collaborate extensively with the other members of the Interdisciplinary team, patients and/or patient's family or significant other(s).
  3. Perform and document nursing process – assessment, diagnosis, planning, implementation, and evaluation of patients receiving care in-person at their defined home.
  4. Provide nursing care, emotional support and education to patients and families.
  5. Assist with wound care, treatments, procedures, and comfort care.
  6. Administer medications and treatments as written in the patient's plan of care to obtain a level of comfort and/or relieve symptoms associated with the patient's diagnosis.

Requirements:

  • Minimum of 2-3 years of hospice nursing experience.
  • Experience working in a home care setting preferred.
  • Excellent assessment skills and clinical judgement.
