About the Role
">We are seeking a highly skilled and compassionate Weekend Physical Therapist (PT) to join our dynamic team. As a key member of our multidisciplinary team, you will work closely with clients in their homes to provide expert care and support.
Responsibilities
">- Assess client rehabilitation needs and develop personalized physical therapy programs to enhance functional mobility, prevent falls, and promote home safety.
- Collaborate with healthcare professionals to ensure comprehensive care and education for clients and their families.
- Provide individualized instruction and guidance to clients and caregivers on rehabilitative techniques and strategies.
- Maintain accurate records and documentation to track progress and outcomes.
Requirements
">- NH PT License; DPT preferred
- Experience in home care or a related field is beneficial but not required; we offer training and development opportunities.
- Reliable transportation and valid driver's license
- Proof of flu vaccination and two-step TB test
- Physical exam within the last year
