As a Clinical Nurse Specialist, you will provide high-quality nursing care to participants in our PACE program, utilizing your expertise in geriatric care to ensure optimal health outcomes.

  About the Role:
  As a Clinical Nurse Specialist, you will work closely with the Clinic Supervisor and other healthcare professionals to assess participants' needs, develop and implement individualized care plans, and provide education and support to participants and their families. Your expertise in geriatric care will be invaluable in ensuring that our participants receive the highest quality care possible.

  Responsibilities:
  Key Responsibilities:
    • Assess participants' physical and behavioral changes and adjust care plans accordingly
    • Provide skilled nursing care to participants as needed in a timely manner
    • Implement preventative and rehabilitative nursing procedures as appropriate for participants' care
    • Participate in IDT meetings and provide input on team planning and problem-solving
    • Document accurately and in a timely manner according to agency standards
  Additional Responsibilities:
    • Provides direction to clinic staff when the Clinic Supervisor is not present
    • Offers nursing consultation to other staff members as needed
    • Assures quality of care by adhering to therapeutic standards and following governing agency regulations

  Requirements:
  To be successful in this role, you will need:
  Essential Qualifications:
    • Graduate of an accredited school of professional nursing
    • Current California Registered Nurse License
    • Minimum 1 year acute or ambulatory care experience working with frail or elderly population
    • Current CPR certification
    • Eligibility for licensure by obtaining required continuing education units
  Preferred Qualifications:
    • Experience working in a PACE environment
    • Knowledge of geriatric care principles and practices
