Job SummaryWe are seeking a highly skilled Music Therapist to join our team at The Care Team Home Health & Hospice. As a Music Therapist, you will play a vital role in providing music therapy services to our patients in their homes or alternate community settings.
Key Responsibilities- Assess and provide individualized music therapy consultations and services to patients and/or family members.
- Develop and implement individualized music therapy plans of care.
- Use creative therapeutic activities to improve or maintain mental and physical function and quality of life.
- Implement music therapy interventions as part of the interdisciplinary plan of care.
- Direct and implement legacy projects for patients/families as identified in the plan of care.
- Direct other care team members in the use of music to achieve individualized care plan goals.
- Document all services provided to hospice patients and families in the clinical record.
- Terminate therapy services when appropriate.
- Support business development activities of the organization.
- Provide in-services to hospitals, nursing homes, and other facilities as requested and determined appropriate.
- Support bereavement activities as needed and appropriate.
- Provide bereavement counseling to family members as identified.
- Participate in and/or facilitate Hospice memorial services.
- Facilitate grief support groups as identified by the IDG.
- Complete documentation timely within 48 hours of visit.
- Serve as the counselor at IDG meetings, participates in on-call rotation, and assists with sign-ups and consents for newly referred patients if needed.
- Educate the hospice staff about the role of music therapy and the use of music therapy intervention with hospice patients and staff.
- Master's degree in Music Therapy or related field.
- Board certification in Music Therapy (MT-BC) or eligible for certification.
- Experience working with hospice patients and families.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Flexibility to work in a variety of settings, including patients' homes and alternate community settings.
