Join our team as a Medical Case Manager RN at the University of New Mexico - Hospitals, where you will play a crucial role in ensuring quality patient outcomes.

Job Summary

We are seeking an experienced Registered Nurse to fill this exciting opportunity. As a Medical Case Manager RN, you will coordinate patient care services and ensure that patients receive the most effective treatment possible within established timeframes. Your expertise will be utilized to manage disease management protocols, initiate assessments, and develop comprehensive plans of care.

Responsibilities
  • Patient-Centered Care: Ensure that all patient-centered medical home or patient-centered specialty practice core expectations are met.
  • Care Coordination: Identify patients requiring case management interventions and coordinate referrals for home health services, oxygen, and equipment.
  • Data Collection and Analysis: Collect, review, and analyze patient data to inform treatment decisions.
  • Assessment and Planning: Assess patient clinical, psychosocial status, and current treatment plans, identifying areas for improvement.
  • Nursing Interventions: Develop and implement individualized nursing plans of care to meet patient needs.
  • Communication and Collaboration: Foster collaboration with healthcare teams, community resources, and patients' families to achieve optimal patient outcomes.
Requirements

To be successful in this role, you will need:

  • A valid RN license in New Mexico or eligibility through reciprocal agreement.
  • CPR for Healthcare/BLS certification within 30 days.
  • One year of direct experience in a healthcare setting.
  • Bilingual English/Keres, Tewa, Tiwa, Towa, Zuni, or Navajo skills (desired).
Working Conditions

This role involves Light Work, exerting up to 20 pounds occasionally, and/or up to 10 pounds frequently, and/or negligible amounts constantly. The physical demand requirements exceed those for Sedentary Work. You may require walking or standing significantly or sitting most of the time, pushing and/or pulling arm or leg controls, and working at a production rate pace entailing constant pushing and/or pulling materials.
