We are seeking a skilled Speech Language Pathologist to join our Home Health team on a PRN basis. As a Speech Therapist, you will play a vital role in evaluating, planning, and implementing individual patient care, educating and counseling patients, and documenting progress.
Key Responsibilities- Evaluate and assess patients timely, based on principles outlined by practice guidelines, evidence-based guidelines, and regulatory standards.
- Carry out treatment plans and modify them to maximize therapeutic benefit and meet discharge criteria.
- Provide initial and ongoing assessments to identify clients' levels of functioning and ability to communicate.
- Develop and revise plans of care in consultation with physicians and other interdisciplinary care team members.
- Prepare clinical and progress notes, and actively participate in multi-disciplinary team meetings, in-services, and case conferences.
- Master's Degree in Speech Language Pathology
- Recent relevant experience preferred
- Speech Language Pathologist (SLP) certification from The American Speech-Language-Hearing Association (ASHA)
- CPR/BLS certification from American Heart Association/American Red Cross/American Safety and Health Institute
- Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP)
- Valid driver's license and ability to drive personal vehicle for RHS business
