Job Overview
Hospice Care Professionals play a vital role in providing compassionate care to patients and their families. As a Registered Nurse with Vitas Healthcare, you will be responsible for delivering high-quality patient care and collaborating with the interdisciplinary team to meet the physical, medical, psychosocial, emotional, and spiritual needs of our patients.

Responsibilities
We are seeking a skilled and compassionate Registered Nurse to join our team. Key responsibilities include:
  • Case Management and Supervision: Follow Vitas policies and standards, and ensure continuous assessment of each patient's needs and implementation of the interdisciplinary plan of care.
  • Nursing Care: Provide primary nursing care for inpatient and continuous care patients as assigned, including palliation of end-of-life symptoms and education on pain management.
  • Quality Improvement: Participate in Quality Assessment and Performance Improvement (QAPI) Program activities, including Performance Improvement Projects (PIPs), gathering clinical data for quality measures, and developing professional growth goals.

Requirements
To be successful in this role, you will need:
  • Licensure: Be currently licensed to practice nursing in the state where the Vitas program is located.
  • Experience: Have a minimum of two years of nursing experience in hospice, home health, or community health within the last five years.
  • Skills: Possess excellent communication, problem-solving, and leadership skills.
