We are seeking a skilled and compassionate Nurse Practitioner to join our Optum House Calls team. As a House Calls Nurse Practitioner, you will have the opportunity to provide high-quality, patient-centered care in the comfort of patients' homes.
Key Responsibilities:- Conduct comprehensive in-home assessments, including medical history, review of systems, medication reconciliation, vital signs, and physical exams.
- Perform evidence-based practice screenings, including point-of-care testing as needed.
- Identify diagnoses and develop care management plans to address patient needs.
- Communicate findings with patients' primary care physicians to ensure seamless care coordination.
- Provide education and support to patients on disease management, medication adherence, and healthy lifestyle choices.
- Active NP license and national certification in Family, Geriatrics, Adult, or Adult-Gerontology Primary Care.
- Ability to complete physical requirements of the job, including lifting, navigating stairs, and sitting/standing/kneeling.
- Reliable transportation for home visits.
- 1+ years of clinical experience in a similar setting.
- Experience in gerontology, cardiology, internal medicine, or endocrinology.
- Home health care or home visit experience.
