About the Role:

We are looking for a compassionate and detail-oriented Physical Therapist Assistant to provide high-quality care to our patients in the comfort of their own homes. As a Physical Therapist Assistant, you will work closely with our Physical Therapists to develop and implement individualized plans of care that promote patient independence and well-being.

Responsibilities:
  • Provide skilled physical therapy services as ordered by the attending physician, including exercises, modalities, and patient education.
  • Assess patient conditions and notify the Physical Therapist of any changes or concerns.
  • Document patient progress and outcomes accurately and in a timely manner.
  • Collaborate with the Physical Therapist to develop and implement plans of care that meet the unique needs of each patient.
  • Provide patient education and support to promote independence and self-care.
Requirements:
  • Current licensure as a Physical Therapist Assistant in the state of employment.
  • Minimum one year of experience as a Physical Therapist Assistant in an acute care setting, with home care experience preferred.
  • Valid driver's license and auto insurance.
