We are seeking a highly motivated and dependable intellectual and developmental disability (IDD) needs assessor in the Chittenden County, Vermont and Windsor County, Vermont territory.

As an IDD needs assessor, you will conduct Supports Intensity Scale – Adult VersionTM (SIS-A)TM assessments with individuals with intellectual and developmental disabilities eligible to receive services from Vermont's Home and Community Based Services waiver.

This is a full-time role, including regular working hours as well as early mornings, evenings, and weekends. Ideal candidates will have a human services background and experience working with intellectual and developmentally disabled populations.

Key responsibilities include:

  • Conducting assessments in a timely and professional manner
  • Communicating effectively with individuals, family members, guardians, and provider staff
  • Representing PCG and the client in a professional manner
  • Documenting assessment information consistently

Required skills and experience include:

  • Ability to interact with individuals and families in a respectful and professional manner
  • Knowledge of Person-Centered Practices
  • Proficiency with Microsoft Office tools and/or other online data entry
  • Dedication to accomplishing goals and overcoming challenges
  • Flexible and self-motivated with intellectual curiosity
  • Excellent oral presentation and written communication skills
  • Solid organization and time management skills

Required education and experience include:

  • Bachelor's degree in a human services related field such as Social Work, Psychology, Nursing, or Sociology
  • At least one year of experience working with individuals with intellectual and developmental disabilities
  • Experience conducting individual assessments with knowledge of behavior rating or psychological testing principles
  • Knowledge and experience with person-planning concepts
  • Prior experience conducting the Supports Intensity Scale assessment is preferred
  • Experience with Home and Community Based Services waivers is preferred
