This is an excellent opportunity to join LHC Group as a highly skilled LPN Patient Care Manager Assistant. As a key member of our home health team, you will play a vital role in coordinating patient care and ensuring the highest level of quality service.

Responsibilities:

  • Assist the Patient Care Manager in coordinating patient care plans and ensure seamless communication with field staff, physicians, and other healthcare professionals.
  • Monitor patient status and facilitate timely lab results and physician notifications.
  • Prepare patient lists and necessary documents for weekly case conferences.
  • Provide direct patient care, as needed, in accordance with scope of practice, plan of care, and physician orders.
  • Participate in clinician documentation review/audits to ensure complete and accurate submission of all documentation by field staff.

Requirements:

To be successful in this role, you must have a current LPN licensure in the state of practice, CPR certification, and a valid driver's license with vehicle insurance. Additionally, you should have at least one year of experience working as an LPN and meet any state-specific requirements.

Salary:

The estimated salary for this position is $65,000 - $80,000 per year, based on industry standards and location.
